I  authorize the Alberni Valley Curling Club to collect, use and disclose  my personal information in order to manage or terminate my relationship  with the club and Curl BC, and to administer benefits that may result  from such relationship.  I understand that the club and Curl BC endeavor  to only collect, use and disclose personal information as is reasonably  necessary to manage or terminate a relationship and administer related  benefits.  I understand that Curl BC’s collection, use and disclosure of  personal information will be governed by the terms of the Curl BC  Internal Privacy Policy and applicable Provincial Privacy Legislation.   For the purposes of this consent, personal information may include some  or all of the following, but is not necessarily limited to: (a) Name   (b) Home address, phone number and email address  (c) Age by category   (d) Gender    (e)…
